Submit a claim

In the event of the death of a loved one, you can submit a claim by sending an email to claims@restassuredplan.co.za with the following documents attached: along with all the accompanying documentation, or you submit the documents here, dont forget to include the filled claim form downlaodable below

    1. ID/Passport of main member (Certified)

    2. ID/Passport of claimant (Certified)

    3. ID/Passport of deceased (Certified)

    4. Proof of banking details (Certified)

    5. Marriage certificate or lobola letter (Certified)

    6. Death certificate (Certified)

    7. Completed Police report (for accidental death or suicide)

    8. BI-1663 or DHA-1663 or BI-1680 (Ensure its signed)

    9. Proof of 6 months premium payments

    9. Completed claim form
