Submit a claim

In the event of the death of a loved one, you can submit a claim by sending an email to claims@restassuredplan.co.za with the following documents attached: along with all the accompanying documentation, or you submit the documents here, dont forget to include the filled claim form downlaodable below

    1. ID/Passport of main member (Certified) (5MB max)

    2. ID/Passport of claimant (Certified) (5MB max)

    3. ID/Passport of deceased (Certified) (5MB max)

    4. Proof of banking details (Certified) (5MB max)

    5. Marriage certificate or lobola letter (Certified) (5MB max)

    6. Death certificate (Certified) (5MB max)

    7. Completed Police report (for accidental death or suicide) (5MB max)

    8. BI-1663 or DHA-1663 or BI-1680 (Ensure its signed) (5MB max)

    9. Proof of 6 months premium payments (5MB max)

    9. Completed claim form (5MB max)
